Summary Description: This key position is responsible for the overall quality, execution of services provided and supervision of the Skilled Nursing Facility (SNF) staff while following established standards of care. Assists the medical providers in coordinating patient care. Functions as a supervisor of professional nursing care and assist as a liaison with Administration, Residents, family, hospital staff and professional health care providers in all the Hospital Districts facilities. Ensures regulatory compliance for the SNF.
Supervision Received: Reports to the Skilled Nursing Facility Administrator.
Responsibilities: 1. Provides management through development and implementation of a philosophy of quality care and of nursing standards, policies, and procedures that are consistent with high quality care and regulatory compliance. 2. Supervises the professional nursing staff as they work in the various departments within the Skilled Nursing Facility including direct patient care, patient treatments, medication administration and other SNF operations. Determines and advocates for staffing levels that promote high quality nursing services. 3. Oversee the completion and continuity of patient records, charts and other pertinent information documented by the professional staff in coordination with the medical records staff. Responsible for clinical chart audits in coordination with nursing, physician, lab, and radiology. Provides reports of those audits to respective departments as requested. Participates in departmental meetings as requested. 4. Oversees the Minimum Data Set (MOS) process and works with Administration regarding reimbursement requirements related to MDS. 5. Assist Administration with developing operational and capital budgets in conjunction with strategic plan development and execution. Monitors budget and expenses monthly. 6. Meets with prospective and current residents and families to ensure positive guest relations. Ensures adherence to individual resident care plans and proactively addresses problems and complaints involving nursing services that affect resident quality of care and satisfaction. 7. Acts as a point of contact for residents' abuse complaints and reports to Wyoming Department of Health within the required 2-hour time limit. 8. Discretionary delegation of key tasks that are critical for optimal function of the nursing team to include but not limited to: Management of PAR levels and ordering of supplies, EMR management, staff orientation and ongoing education, etc. 9. Assists the professional nurses and primary health care providers with patient care, patient examinations, interventions, and treatment procedures as needed. Ensures that residents' rights are protected and that the residents are free from abuse and neglect. 10. Oversees medical supply and medication formularies and collaborates with medical directors regarding such. Reviews medication and supply formularies annually and on an as needed basis in conjunction with medical director. Assists pharmacists with regulated patient review and ensures that recommendations are implemented. 11. Is responsible for scheduling the professional nursing staff coverage for the SNF, approving leave requests, and verifying timecard accuracy prior for submission to payroll and ensures posting of staff per regulatory requirements. 12. Is responsible for overseeing initial training and continuing education of the nursing staff to promote quality, continuity, and safe care of the residents. Develops and maintains Nursing Competency plan in collaboration with the Medical Directors. 13. Is responsible for annual employee performance evaluations for the professional nursing staff, including documentation of such in the employee's personnel file. Is responsible for verifying the continued competencies and licensures of all professional nursing staff. 14. Positively represents the Hospital District at community, state and regional meetings associated with SNF excellence. Continually works to assure collaboration and maintenance of standard of care. 15. Provides the necessary oversight so that SNF and infection control policy/procedures and practices are of the highest standard based on local, state, and national certifying and governing bodies. Is an ad hoc member of the Policy/Procedure Review Committee and SNF Safety Committee. 16. Responsible for developing and/or maintaining nursing policies and procedures that ensure consistent quality and safety while meeting regulatory requirements. 17. Serves as the SNF representative for the Hospital Districts Quality Performance Improvement Committee. Is responsible for collaboration with all Hospital departments to assure that quality clinical standards, standards of practice and best practices are attained and maintained. 18. Participates in and provides leadership for organizational disaster preparedness and resident safety initiatives in collaboration with local, state, and regional entities. 19. Delegates and monitors monthly safety inspections on management of outdated medications, supplies, and/or equipment in collaboration with the Safety Coordinator. 20. Assesses need for equipment and supplies. Assists with product selection and management if changes are needed. Updates inventory PAR levels as needed. 21. Provides education on new equipment, products and/or medications in collaboration with Nursing and Medical team. 22. Maintains certification in all laboratory procedures as documented in the laboratory policy manual and works cooperatively with the Laboratory Director to maintain staff proficiencies in venipunctures and specimen collection procedures. 23. Effectively and efficiently leads teams and communicates to all members of the Hospital District. 24. Works cooperatively with the Medical Director, SNF Director, and District Administrator to comply with OSHA and Federal Regulations. 25. Ad hoc member of the meaningful use committee
Qualifications: 1. MSN preferred. BSN, AON, and Diploma graduates accepted. 2. Current State of Wyoming RN License without restrictions. 3. Certified Director of Nursing Administration (CDONA) Certification (may obtain after hire) 4. Minimum of 5 years professional nursing experience. 5. Minimum of 3 years' experience in a managerial role. 6. Current certifications in BLS.
Knowledge, Skills and Abilities: 1. Knowledge of professional nursing theory, regulations, and practices to give and evaluate patient care and the delivery of patient care by professional nursing staff. 2. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment for the medical staff and patients. 3. Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care. 4. Ability to foster a positive, resident-centered culture. 5. Skill in identifying problems and recommending solutions as a manager. 6. Skill in preparing/maintaining records, writing reports and responding to correspondence as a nurse manager. 7. Ability to supervise a "health care team." 8. Ability to maintain and evaluate quality control standards. 9. Ability to react calmly and effectively in managing emergency situations. 10. Ability to communicate clearly and effectively as a nurse manager.
Working Conditions: 1. Subject to varying situations including ability to understand and respond appropriately, effectively, and sensitively to special population groups, including those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, etc. 2. Frequent contact with a variety of people including prisoners and people with mental, emotional, and physical disabilities. 3. Medical office, exam room, laboratory, radiology, emergency room and professional practice settings. Frequent exposure to communicable diseases and other conditions common to rural clinic environments. 4. The work environment involves exposure to unusual elements, potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. 5. May require travel between District sites.
Physical Demands: Varied activities include standing, walking, reaching, bending, and lifting. Requires full range of body motion including handling and lifting patients. Also requires manual and finger dexterity, and eye-hand coordination. It requires standing and walking for extensive periods of time. Occasionally lift and carry items weighing up to fifty pounds. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.
